Diabetes Partnership of Cleveland is a local and independent nonprofit organization serving our community since 1954.

Diabetes Partnership of Cleveland works to reduce the number of people with prediabetes, diabetes and diabetes complication through prevention services and supports those impacted to better manage and control their health.
