MetroHealth Comprehensive Care Program
MetroHealth makes it easy for patients with special healthcare needs and their families to see a primary care team solely focused on caring for people with complex medical conditions in one, convenient location.
The Comprehensive Care Program cares for the whole patient, and not just specific conditions. Our Care Teams help families address all of the issues that can impact a patientrsquos wellness: their physical health, nutrition, mental wellbeing, social service needs, and much more. We take time to get to know our patients well, and patients and their families feel comforted knowing that a person who understands their medical history is just a phone call or click away.
MetroHealthrsquos program is the first one of its kind in Northeast Ohio to offer a holistic approach to primary care for both children and adults with special health care needs.
A Medical Home for Patients
Our program has been caring for patients with special needs for more than 50 years. In 2011, the Comprehensive Care Program was the first practice at MetroHealth to be awarded a National Committee for Quality Assurance (NCQA) Level 3 Recognition, the highest possible level for Excellence of Care.
We see patients with:
• Cerebral Palsy
• Conduct and behavior concerns
• Craniofacial disorders, such as cleft lip/palate
and disorders of the head, neck, ear and or nose
• Downrsquos Syndrome and other
chromosomal or genetic disorders
• Intellectual developmental delays
and cognitive delays
• Metabolic disorders
• Spina Bifida
In addition to primary care, we offer onsite access to specialists and other services such as, nutrition services provided by a registered dietician. If a patient needs psychological services, we can arrange for an appointment with a mental health specialist. And, because we know that it is easier for families to receive care in the same location and from the same doctors, we see family members without special needs in our program, as well.
