Hospice care is a physician-directed program of medical, nursing, social, spiritual, and emotional support services for those persons facing the end of life. The focus is on providing relief from pain and other symptoms and creating an atmosphere where quality of life is honored for both the patient and family.
Service Area
Hospice Palliative Care of Greater Wayne County is a non-profit agency that serves those who live in Holmes, Medina and Wayne Counties and surrounding areas. Hospice care can be provided in the patientrsquos home, an assisted living setting, a hospital or a nursing home.
Referral Process
Physicians, healthcare providers, families, friends and neighbors can make a referral by calling the Hospice office. Hospice staff will provide information about hospice care and offer an opportunity for the patient and family to discuss their individual needs.
